It’s safe to say that over the past few decades, the breast implant trends and overarching breast and body aesthetics popularized in culture have changed dramatically. While bigger was once better, the age of Ozempic and GLP-1 weight loss drugs has prompted a shift towards a preference for a more toned, fit appearance, with trends like “yoga boobs” often taking the place of double-D implants in recent years.
Looking at aesthetic trends from a bird’s-eye view would make it seem like the breast implant preferences today skew smaller, but a closer look at the regional differences in what’s most popular among breast augmentation patients makes it abundantly clear that the bust trends of today are more nuanced than we might have thought. To get a better idea of how breast implant trends are shaping up in 2025, we tapped expert plastic surgeons from across the country to discover the breast implant size their patients are requesting most often, and the differences from coast to coast were enlightening.
South/Southwest
For Tucson, AZ plastic surgeon Raman Mahabir, MD, the average breast implant size requested by patients is around 350cc, but the relative size from patient to patient is what makes the biggest difference. “In a larger person, 350cc is not very much and on a smaller person, 350cc could be too much,” he explains. “What’s really interesting is that, in the last five years, there’s been a trend towards smaller implants. We use the patient’s body measurements to give them a range of implants that will fit their body and still look very natural.” Las Vegas plastic surgeon Michael C. Edwards has noticed a similar trend in his practice, noting around 300cc as his average breast implant size among patients.
Emphasizing further how the natural look has taken charge in his Arizona-based practice, Dr. Mahabir explains that “Five years ago, it was very common for patients to pick implants at the largest end of that range and sometimes ask for a little more. Now what we see is patients asking for something either in the middle or at the smaller end of that range.” As a result, Dr. Mahabir has noticed “more natural, longer-lasting results.”
“In my practice as of late, the average implant size has shifted from 350-400cc to closer to 250-300cc,” says Louisville, KY plastic surgeon Bradley Calobrace, MD. Dr. Calobrace has also experienced an uptick in patients choosing explantation or lifts without implants, which he credits to “an increase in implant hesitancy and implant fatigue.”
Midwest
For Chicago plastic surgeon Michael Horn, MD, smaller implants are also on the rise. “Currently in my practice, patients are choosing smaller, more natural-looking breast implant sizes than they were five years ago,” he explains. “Using computer imaging, the patients can get an accurate view of how different sizes actually appear in their own bodies. As a result, patients now commonly choose in the 250- to 350-cc size range when previously patients were drawn to the 350- to 500-cc size range.”
East Coast
On the East Coast, even smaller implants are leading the charge, with New York plastic surgeon B. Aviva Preminger, MD noting primarily 200-cc to 250-cc implants being requested among her patients. “Most women are seeking a natural, proportionate look that complements their frame rather than an exaggerated result,” she explains. “For petite patients, that often means leaning toward the lower end of the range, while taller or broader-framed women may go slightly larger. The overarching theme is harmony and balance rather than simply ‘going big.’ We have so many more options now in terms of smaller implants.”
West Coast
On the West Coast, implant trends are still tending to skew a bit larger, though not by much. For La Jolla, CA plastic surgeon Robert Singer, MD, 300-cc to 350-cc implants tend to be the norm amongst patients, keeping to a similar trend as that of the Southwest doctors, though noticeably larger than that of the East Coast, based on Dr. Preminger’s experience.
For Newport Beach, CA plastic surgeon Sanjay Grover, MD the average implant size among patients stays around 300cc as well. “The average implant size requested has decreased over the years for several reasons,” Dr. Grover explains. “One main reason is the change in style or fashion. Gone are the days of the ’80s Baywatch-style breasts. Today we are seeing fashion changes that are trending towards the more ‘ballerina boob’ or ‘Barbie boob’ fashion, where one is just getting enough volume to eliminate the need for padded bras,” he explains. “Additionally, women (and men) today are becoming even more health- and fitness-conscious than years ago, and many are working out and active and do not want their breast implants to impede on their ability to do athletic activities.”
Northwest
Hideout, UT plastic surgeon Mark Jewell, MD says his “workhorse implant” is the Allergan Inspira SSF—known for its soft touch gel and midrange stiffness—but the average size among his patients is determined by measuring the individual’s breast base width. “Implant sizing in my practice typically ranges from 265cc to 365cc,” he explains.” Occasionally, I will go as large as 405cc,” he adds, noting that “Implants are typically placed in the retromammary sub-fascial location to avoid animation deformity and late term waterfall deformity.”
